Candace Hack APRN, PMHNP-BC is a Psychiatric Mental Health Nurse Practitioner who is board certified through the American Nurses Credentialing Center. She earned her BSN in Nursing at Murray State University. She worked in long-term care and behavioral health before deciding to further her education. She went on to further her education specializing in Psychiatric Mental Health Nurse Practitioner at Northern Kentucky University.

Our Goals:

A primary goal of our providers is to improve your mental health. Your psychiatric provider will meet with you first to perform a thorough evaluation, by listening to your concerns and getting to know you. The provider will formulate a treatment plan based on the assessment that works best for you. Pathway Mental Health uses a combination of therapy with medications to target symptoms. Prescribing medications when indicated and using the least number and the lowest effective doses.

Pathway Mental Health is dedicated to providing you with the highest quality, cost effective psychiatric care. We are in-network with many commercial insurances. Those insurances that we are out of network with, we will be happy to give you a superbill to submit to your insurance for possible reimbursement. We cannot guarantee your insurance will reimburse you, that is going to depend on your plan.

Payment for services rendered - including co-pays/coinsurances are due at time of appointment.
